We have an opportunity for 1 x part-time (22.5 hours per week), Care Coordinator - Palliative Care based at Rectory Road Medical Centre, based in Staveley, Chesterfield.
Care co-ordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide co-ordination and navigation of care and support across health and care services. This role will focus on supporting patients who are on the Palliative Care register new or longstanding. To ensure their care journey across health and well being is joined up and suitable for their health and care needs
An opportunity has arisen to join our growing team at North Derbyshire PCN supporting two member practices. We are looking for an enthusiastic part-time Care Coordinator to work within in our PCN practices in the Chesterfield area focusing on supporting patients who are Palliative. Whilst you will be based at our Rectory Road site, you will be looking after patients across both Member Practices of the PCN.
The successful candidates main responsibilities will be to:
· Provide co-ordination and navigation for people and their carers across health and care services. Helping to ensure patients receive a joined-up service and the appropriate support from the right person at the right time.
This role will require excellent IT skills, good prioritisation skills and an understanding of General Practice processes or relevant transferable skills.

To work within the PCN team with a focus on achieving excellent patient care via detailed clinical administrative support, coordination and liaison with patients and professionals. The successful candidates main responsibilities will be to provide excellent care navigation support to patients who are on the Palliative list. Ensuring all patients have good access to care, in a joined up and holistic approach.
